• TOR with RLE
• BSN Diploma
• Certificate of Good Moral Character
• NBI clearance (current)
• Birth Certificate (NSO)
• 2 pieces 2×2 colored picture (white background)
• brown envelope (place photocopies here, right your name at the back)

Reminders:

-Training Fee (3month training program) – verify first to nsg office because I’m not sure if this is applicable

Go placidly amid the noise and the haste,
and remember what peace there may be in silence.
